There are always new and improved techniques and technology in medicine. This is particularly true when it comes to rhinology “the study of the nose”. One needs to be well versed in Facial Plastic Surgery, Allergy and Immunology, Headaches and Facial Pain, and of course the Medical and Surgical treatment of Sinus and Nasal disorders.   Xolair (Omalizumab) is currently in a phase IV study for the treatment of chronic rhinosinusitis.
• Sublingual Immunotherapy “Allergy Drops” Continue to Gain Credibility
  Ongoing FDA trials and multiple new studies continue to confirm the efficacy of Sublingual Immunotherapy. Probably partly because of economic pressures Sublingual Immunotherapy continues to face more resistance in the U.S. It is important to note that no one is advocating drops over shots. High dose subcutaneous shots with standardized antigens is clearly the “gold standard”, however, for those not candidates for shots we feel that sublingual immunotherapy may be a good alternative.
• Nasal Inhalation of Dornase Alfa (Pulmozyme) in Patients With Cystic Fibrosis and Chronic Rhinosinusitis
  CF patients tend to be the most refractory sinus patients.
• Probiotics
  “Good Bacteria” manufacture B vitamins, aid in digestion, and keep harmful bacteria and fungi at bay.
• Macrolides used as immunomodulators in the treatment of Chronic Rhinosinusitis
  Using lower dose macrolides for extended duration in the treatment of Chronic Rhinosinusitis is extremely helpful for select patients.
• New means of topically treating Chronic Rhinosinusitis
  The topical delivery of medications is often greatly facilitated by surgical intervention. Using topical antibiotics, antifungals, steroids, and mucolytics is often the most effective long term treatment. A number of new devices facilitates delivery of these agents.
